Collecting and Using Personal Information
At USA Managed Care Organization ® (USA Managed Care Organization ® and its affiliated or subsidiary companies), we are very aware of the privacy concerns of our customers and the members of the general public who visit us at our USA MCO sites. In fact, we share your concerns and we want you to know that we are firmly committed to ensuring that when you visit and navigate our sites and when you communicate with us via our sites, we will not collect personal information about you, unless you provide us that information voluntarily. If you do elect to provide personal information to us voluntarily, in no case will we sell, license or transmit for any reason that information outside of USA MCO, unless required to do so by legal, judicial or governmental proceedings, or unless we do so in accordance with any authorization you have given us. Also, in some cases, we may disclose information to our contractors or agents for the purpose of enabling them to perform certain functions for us.
By “personal information,” we mean data that is unique to an individual, such as a name, address, social security number, or telephone number. Other Information; “Cookies”
You should also be aware that when you visit our Web site, we collect certain information that does not identify you personally, but provides us with “usage data,” such as the number of visitors we receive, what pages are visited most often, and what browser and domain name you are using.
