Arkansas Standard Application: For Providers in the State of Arkansas. The Standard Application must be included along with the appropriate agreement below.
Provider Agreement: Use this agreement if you are an individual provider or a provider group.
IPA Agreement: Use this agreement if you are a group of providers accepting delegated credentialing.
Ancillary Agreement: Use this agreement for facilities providing Outpatient and other Ancillary Services.
